Internship Application Forms

The Intern Application process is lengthy; therefore, you are encouraged to begin early.  All of the following application materials must be submitted: (the forms below are in portable document format (.PDF), and require Adobe’s free Acrobat Reader to view).

1.) Internship Application

  • Must be typed and professional in appearance, no handwritten applications will be accepted;
  • Must include the correct name, address, and phone number of the person who is to receive your application;
  • Must be signed and dated.

2.) Biographical Sketch (example)

  • Must be typed and professional in appearance;
  • Must be written in the third person;
  • Should include people and events which had an impact on your career decisions, include any experiences, competencies, and interests that may be helpful to your internship.

3.) Resume (example)

  • Must be typed and professional in appearance;
  • Should include personal information, education, employment experiences, volunteer experiences, honors & recognition’s, etc.;
  • Should be no more than three pages in length.

4.) Letters of Recommendation

  • Three letters of recommendation are required;
  • Letters of recommendation should be from APK faculty or your supervisory committee members;
  • Letters of recommendation from friends or family will not be accepted.

5.) Transcripts – must be picked up from Criser Hall.

6.)  CPR

  • Must be CPR certified the first through the last day of internship;
  • Provide copy of your card, front and back.

8.) Assembly of Application Packet

  • Assemble all of the above information in order, #’s 1-7;
  • Copy and staple;
  • Paperclip the original and copied packets together.

9.) Submission and Approval of Application Packet

  • Submit both (original and copy) packets to Mike Balkcom in room 100 FLG no later than 4:00 p.m. on the deadline date, along with an approved Grad Contract.

*FAILURE TO ABIDE BY ANY OF THE POLICIES ABOVE MAY RESULT IN YOUR BEING UNABLE TO INTERN THE SEMESTER OF YOUR CHOICE.

Attached you will find information and forms regarding your internship.  Be sure to review each form and return them within the requested time frame.  This packet includes the following forms:

  • Midterm Progress Report: to be completed by you, signed by your site supervisor and returned to your university supervisor by the end of the 8th week of your internship
  • Evaluation form: to be completed by you and the site supervisor, signed by the site supervisor and returned to your advisor the end of the 14th week of your internship.
  • Grade Evaluation: to be completed by the site supervisor and returned to your advisor the end of the 14th week of your internship.
